Insurance policies generally cover only a portion of the total treatment cost. It is your responsibility to pay any balance not paid by your insurance company. Unless other arrangements have been made, we ask that you pay your portion of the bill at the time of treatment.
Our staff will gladly process insurance claims as a convenience to you. Please bring the necessary forms with you on your first visit.
We are a provider for several insurance plans which include the following insurance companies: Ameritas, Blue Cross Blue Shield, Delta Dental, Lincoln Financial, Metlife, Principal, and UMR.
We also accept Care Credit and all major credit cards, checks and cash.
We offer a membership plan to our office as an alternative to insurance called illumisure for your dental needs and hygiene. Please inquire if you have any questions regarding insurance or payment options.
